Assistance Provided: Approximately 12 $1,000 – $3,000 scholarships are awarded every year for tuition.
Eligibility Requirements: Applicant must be a graduating high school senior, community college and four‐year university, and graduate students who is a leukemia or lymphoma patient or is the immediate family member of a leukemia or lymphoma patient and resided in the same household with the leukemia or lymphoma patient during their treatment. A letter from the treating doctor must be provided to certify that the applicant or applicant’s sibling or parent has/had leukemia or lymphoma, or a Certificate of Death stating leukemia or lymphoma as the cause of death. Applicant intends to enroll full-time at a college or university. Applicant has a minimum 3.0 GPA. Applicant demonstrates financial need. Applicant who is pursuing a health or nursing related major may receive priority.

Application Process: Application process consists of: application, color photograph, doctor’s letter or death certificate, 2013 FAFSA form (EFC worksheet only), 2013 Federal Tax return, 2 letters of recommendation, SAT and/or ACT scores, high school or college transcript, essay between 600 and 800 words, letter of special circumstance (if applicable). 2014 application deadline is April 15, 2014. Award will be distributed by July 31, 2014.
