Georgia:

The Lydia Project Emergency Relief Services

Assistance Provided: Maximum of $500 for past due rent and/or mortgage and/or utility bills. Maximum of $150 for prescription drug assistance at Barney’s Pharmacy.

Eligibility Requirements:  Applicants must be female cancer patients in active treatment who reside in Aiken, Burke, Columbia, McDuffie, or Richmond Counties.

Application Procedure: Doctor must complete and sign an Emergency Relief Services referral form and fax it to The Lydia Project.

Illinois:

No Wooden Nickels- MedStart-5 Program

Assistance Provided: Financial assistance for cancer patients in Illinois for treatment-related out-of-pocket expenses, such as transportation to treatment and co-payments.

 

Indiana:

Touch Inc.

Assistance Provided: Financial assistance for day-to-day non-medical expenses for cancer patients, such as, transportation, utilities, child care, housing mortgage or rent. TOUCH DOES NOT pay credit card bills. There is a fixed monthly limit of $250 and a limit of 12 months of assistance.Checks will be paid directly to the vendor, not the cancer patient, and TOUCH requires a written statement or verification from the vendor of the amount due.


Eligibility Requirements:
Residency: Applicants must be residents of Vanderburgh and Warrick counties in the state of Indiana. A driver’s license or a copy of an apartment lease or bill with your name and address to verify resident status may be required.
Medical: Applicants must have been diagnosed with cancer within the previous five months, or currently undergoing medical treatments for the cancer.  TOUCH will need diagnosis and treatment information from your medical provider and must have proof of your consent to obtain this information. You must submit a signed letter from your oncologist/physician verifying your current diagnosis and treatment plan.
Financial:  Applicants must meet the following requirements.
1. Income. Applicant’s total household income from all sources at the time of application must be less than the Area Median Income for the county in which he/she lives.
2. Assets. Cash or liquid assets (i.e., CDs, stocks, mutual funds) must be less than expenses for the duration of treatment AND the applicant may not own secondary real property or other liquid real or personal property.
3. Assistance in paying mortgage. The value of applicant’s home must be less than the median home sales price for the county in which the applicant lives. In order for TOUCH to make this determination, the applicant must  provide a copy of the current year’s property tax assessment.

 

Massachusetts:

Adam Bullen Memorial Foundation

Assistance Provided: Financial assistance for adult and pediatric cancer patients in Massachusetts.

The FLY Foundation

Assistance Provided: Financial assistance for young adult cancer patients, ages 19-39, in Massachusetts, for medical expenses and living expenses.

 

Minnesota:

Angel Foundation

Assistance Provided: Financial assistance for adult cancer patients over the age of 18, who are in active treatment.  Financial assistance covers day-to-day non-medical expenses, such as rent,  utilities, grocery store cards, and gas cards.
The maximum amount any applicant can receive is $600, and the money must be used within 90 days of approval. Checks are made payable directly to the vendors, but are sent to the applicant to forward to the vendor. Grocery store cards and gas cards are sent directly to the applicant. Applicants can only receive assistance once in a 24 month period.


Eligibility Requirements:
In addition, applicants must reside or is being treated in the seven-county metro area of Minnesota–Ramsey, Dakota, Washington, Anoka, Hennepin, Carver and Scott, and must meet the financial guidelines set by Angel Foundation.
