Cancer Care Co-payment Assistance

Assistance Provided:       Co-payment assistance for those who are eligible based on income, diagnosis and insurance coverage.

Eligibility Requirements:   Applicant’s individual or family income can be up to 4 times the Federal Poverty Level.  Applicant must be in active treatment in the US for one of the following cancers: metastatic breast cancer, colon or colorectal cancer, gastric cancer, non-small cell lung cancer, pancreatic cancer, or renal cell cancer. Applicant must be covered by insurance through a private plan, employer health plan, or Medicare Part B, Medicare Part D, or Medicare Advantage Plan.

Co-Pay Relief Program

Assistance Provided: Co-payment assistance for those who are eligible based on income, diagnosis and insurance coverage.

Eligibility Requirements: Applicant’s household income must be 500% or less of the Federal Poverty Level. Applicant must be in active treatment for one of the following diseases: non-small cell lung cancers, cutaneous t-cell lymphoma, multiple myeloma, myelodysplastic syndrome (and other pre-leukemia diseases), osteoporosis, pain, hepatitis C, renal cell carcinoma, colon cancer, prostate cancer, and electrolyte imbalance. Applicant must have health insurance coverage.

Healthwell Foundation

Assistance Provided: Financial assistance for those who are eligible based on income, diagnosis, and insurance coverage for  coinsurance, copayments, healthcare premiums, and deductibles for specified treatments and/or medications.

Eligibility Requirements: Applicant’s individual or family income can be up to 4 times the Federal Poverty Level. Applicant must have health insurance coverage. Applicants should refer to http://www.healthwellfoundation.org/diseases-and-medications  to determine if Healthwell has a fund for their diagnosis.

Patient Access Network Foundation

Assistance Provided: Co-payment assistance for those who are eligible based on diagnosis, income, and health insurance coverage.

Eligibility Requirements: Applicant must reside and receive treatment in the United States. Applicant must have health insurance coverage. Applicant’s income must be below a percentage of the Federal Poverty Level, which varies based on disease fund. Applicant’s disease must be covered by a PAN Fund.

GSK Access

Assistance Provided: GlaxoSmithKline prescription medications for free to Medicare Part D Prescription Drug Plan enrollees who meet the eligibility requirements.

Eligibility Requirements: Eligibility is based on US residency, annual household income, and proof that the applicant has spent $600 or more for prescription medicines for the year.

Assistance Provided: Financial assistance for eligible medications, co-payments, insurance premiums, diagnostic testing, and  travel for consultations with disease specialists for US legal resident or US citizen patients who qualify based on disease and income.
