Leukemia & Lymphoma Financial Assistance

Bone Marrow Foundation Patient Aid Program

Assistance Provided: Maximum of $1,000 grant to assist transplant patients with transplant-related costs, including but not limited to, donor searches, bone marrow and stem cell collection, cord blood banking, housing, child-care.

Eligibility Requirements: Applicants must be affiliated with a certified transplant center and either awaiting transplant or receiving post-transplant care.

Application Process: Social worker or nurse completing application must mail original completed application to BMF. Physician at transplant center must verify diagnosis and treatment.

 Leukemia & Lymphoma Society- Patient Financial Aid

Assistance Provided: $100 a year per applicant for expenses

Eligibility Requirements: Applicant must be a US resident or US military personnel stationed abroad and in active treatment or ongoing medical follow-up care for a blood cancer. Applicant must show financial need.

Application Process: Applicant must apply annually after June 30th to receive assistance. Applicant’s doctor must complete and sign application form.

Leukemia & Lymphoma Society- Co-payment Assistance Program

Assistance Provided: Financial assistance for health insurance payments and co-payments. Assistance is pro-rata based on “patient specific year”.

Eligibility Requirements: Applicant must be a US resident or US military personnel stationed abroad and in active treatment or ongoing medical follow-up care for a blood cancer. Applicant must show financial need.

Application Process: Applicant must apply on the LLS website by first registering for an account (A valid e-mail address is required). Application period is open after July 1st annually. However, applicants are assigned a “patient specific year” for renewal applications. Application includes on-line application form, financial information, doctor’s form, and a copy of the patient’s insurance card. Applicants should refer to LLS webinars prior to submitting an application

Leukemia Research Foundation

(residing within 100 miles of Chicago or in Illinois)

Assistance Provided: Financial assistance for treatment-related expenses

Eligibility Requirements: Leukemia or lymphoma diagnosis and residency in Illinois or within 10o miles of Chicago.

Application Process: Application consists of application, written diagnosis from doctor, and past 2 years federal tax returns (supporting schedules not required.

Lymphoma Research Foundation Patient Aid Grant Program

Assistance Provided: Limited financial assistance for expenses related to lymphoma treatment through Patient Aid Grant Program, such as medical bills, transportation to and from treatment, lodging during treatment, and childcare during treatment.  Everyday living expenses, such as food, rent, and utility bills are NOT covered.

Eligibility Requirements: Applicants must be applying for the first time and in active treatment. Applicants must be legal residents of the United States.  Applicants must also show financial need, maximum assets are: $15,000 for single individual, $20,000 for couple, $25,000 for family.

Lymphoma Research Foundation T- Cell Transportation Assistance Fund

Assistance Provided: Financial assistance for transportation costs for lymphoma patients in active treatment.

Eligibility Requirements: Applicants must be legal U.S. residents and in active treatment for lymphoma as certified by physician. Applicants must also demonstrate financial need.
